This is believed to be a "Malfunction Overall Test" (MAL OAT) report generated from simulated flight data as preparation for the launch of the Skylab 3 mission (AS-207), which was the second manned mission to the Skylab space station. This test pertains to the Saturn IB rocket, and thus the report is associated with the Launch Vehicle Digital Computer (LVDC) rather than the Apollo Guidance Computer (AGC).

These are the Apollo 8 crew logs, as filled out by the crew, plus supplementary information about the mission.  I am told that NASA made about 15 sets of photocopies like this (with this scan being from set #15) for use by NASA executives, on heavy-stock 11" by 17" paper, as reference data for later missions.

This is the lunar surface checklist for the Apollo 17 EVA-1 ... i.e., the first excursion from the Lunar Module onto the Moon's surface. The checklist was still attached to the spacesuit glove when photographed, so there are some interesting views of the glove as well.

"FCC" stands for Flight Control Computer, from the Instrumentation Unit of Saturn V and Saturn IB rockets used in the Apollo missions of the late 1960's and early 1970's.  In modern terms, the word "computer" is a misnomer: The FCC was not a digital computer, nor does it run software.  You can think of it as an analog computer whose primary function was to reduce attitude errors in the rocket: i.e., to help prevent it from flopping over in flight.
